Rear of turret, hull machine gun, and the KV did have an option of a commander's machine gun on a mount. Looking up the KV-1, it also has a coaxial machine gun mounted next to the gun barrel. So it actually could have four machine guns.

Yep, that on the rear of the turret is one of the two I count (the other being the on the left side front). It's the third one I cannot figure out.

Since I don't see the commander's machine gun mounted to the hatch ring I spoke about, it would have to be coaxial mounted with the main gun barrel which was the fourth one I mentioned....
